Free health assessments offer for new Mid-Corporate clients FAQ

These terms and conditions apply to our limited period Health Assessment offer for new customers on an AXA Health Mid-Corporate Plan.

Please read these terms and conditions carefully and make sure you understand them. 

The offer is for up to three (3) free onsite Health Assessment Days for new Mid-Corporate Advance customers of AXA PPP healthcare Limited (“AXA Health”) (the “Offer”). 

The promoter of the Offer is AXA Health Limited (registered number 12839134), whose registered office is at 20 Gracechurch Street, London EC3V 0BG, (“we” or “us” or the “Promoter”). We are a service company to AXA Health, and both companies are part of the AXA Group (as defined below).

In addition to these terms and conditions, to use the Offer you will be asked to accept the AXA Health Services Limited terms and conditions and the AXA Health Services Limited privacy policy (the “AHSL Ts&Cs”). 

AXA Group” means AXA UK Plc and its subsidiary companies, any holding company of AXA UK Plc and any subsidiary companies of such holding company, as such terms are defined in section 1159 of the Companies Act 2006. 

Business Day” means any day other than a Saturday, Sunday or bank or other public holiday in England and Wales, Northern Ireland or Scotland. 

Broker” means an agent who is authorised to act in the interests of AXA Group and sell an Eligible Plan to you.   

Effective Date” means the day your Eligible Plan takes effect. 

Eligible Plan” means a new Mid-Corporate Advance plan underwritten by AXA PPP healthcare Limited. 

Health Assessment Day” means a Business Day upon which AXA Health Services Limited (or any of its subcontractors) will conduct up to a maximum of 24 ‘Know Your Numbers’ health assessments to your employees at your business premise. 

Mid-Corporate” means AXA Health’s corporate plan for groups between 65 and 250 employees. 

Offer Period” means the period starting on 1 January 2026 and ending on 30 June 2026 inclusive.  

Redemption Period” means the period of three (3) months commencing on the Effective Date. 

Initial Order Form” means the form received by you from AXA Health titled ‘Ad Hoc Wellbeing Services Initial Order Form’. 

Eligibility for the Offer 

  1. The Offer is only made available to a Mid-Corporate entity who meets the criteria listed in these terms and conditions (“you”,your”).   

      2. To take advantage of this Offer, you must: 

          a. not currently have a policy or be renewing a policy with AXA Health;  

          b. purchase an Eligible Plan with AXA Health (directly or via a Broker) within the Offer Period; 

          c. the purchased Eligible Plan must take effect during the Offer Period; and 

          d. have an active Eligible Plan on the date the Health Assessment Day(s) takes place. 

Claiming the Offer 

3. In order to redeem this Offer you will be asked to accept the AXA Health Services Limited terms and conditions and the AXA Health Services Limited privacy policy (the “AHSL Ts&Cs”) as per your Initial Order Form. 

 4. Subject to these terms and conditions, on redemption of this Offer you shall  receive one (1) Health Assessment Day for each 100 employees covered under the Eligible Plan as follows:

5. The Offer is limited to a maximum of 250 employees covered under the Eligible Plan.  

6. You can exchange the Health Assessment Day(s) for alternative AXA health assessments or wellbeing services of the same value.   

7. The Offer must be redeemed within the Redemption Period and the Health Assessment Day(s) must take place before 31 December 2026.  

8. AXA Health and you shall agree the date of the relevant Health Assessment Day(s) at least six (6) weeks in advance of the relevant date and you shall give AXA Health at least twenty (20) days’ notice in the event you wish to reschedule a booked Health Assessment Day. 

9. A Health Assessment Day may be cancelled or rescheduled as advised by your AXA Health Account Manager.  

10. Under Schedule 1 of the AHSL Ts&Cs the cost of travel and/or accommodation may need to be added, and such details will be confirmed in confirmation emails following the booking of the Health Assessment Day. As part of this Offer, AXA Health will cover such travel costs up to a maximum of £150. 

General information 

11. AXA Health reserves the right to suspend, withdraw, cancel or alter the Offer without prior notice at any time. However, this will not affect any Offers which have already been used to schedule a Health Assessment Day at the time of the change.  

12. This Offer is non-transferable and has no cash alternative.  

13. This Offer must not be used in conjunction with any other offer.   

14. Any personal data collected will be stored and processed for the purposes of administering and assessing the Offer. AXA Health will comply with all requirements of the Data Protection Act 2018. 

15. AXA Health is not liable for any Offer that is unused unless these are caused by AXA Health’s negligence. 

16. By taking advantage of this Offer, you agree that neither AXA Health nor any member of the AXA Group are liable for any loss, damage or personal injury whatsoever suffered or sustained in connection with this Offer or use of the Offer (including but not limited to direct, indirect or consequential losses, loss of profit, income or goodwill), save for any liability which cannot be excluded by law (including fraud, gross negligence or for death or personal injury caused by our or an AXA Group affiliates’ negligence). 

17. AXA Health reserves the right to revise these terms and conditions at any time without giving prior notice and by continuing to take part in the Offer subsequent to any revision of these terms & conditions, you shall be deemed to have agreed to any such new or amended terms. 

18. If any of these clauses should be determined to be illegal, invalid or otherwise unenforceable then it shall be severed and deleted from these terms and the remaining clauses shall survive and remain in full force and effect.

20. These terms and conditions shall be governed and construed in accordance with English Law and any disputes arising from these terms and conditions shall be subject to the exclusive jurisdiction of the Courts of England and Wales.
